Medical Release: As a parent/guardian, I give my permission for the above minor to attend Awana activities conducted by Hampton FBC AWANA clubs including regular club nights and any special activities from July 27, 2022 to June 1, 2023. I understand my child/children may participate in physical activities such as those held during game time. As with any physical activity, there is risk of injury. I full accept this risk and consent to the treatment of any minor injuries of my child/children. I authorize treatment under the direction of any licensed physician of the above minor in the event of a medical emergency which in the opinion of the attending physician my endanger his or her life, cause disfigurement, physical impairment or undue discomfort if delayed. I assume responsibility for all costs connected to any accident or treatment of my child. This authority is granted after reasonable effort has been made to reach me by phone. I will hold harmless and indemnify AWANA and the Church and their officers, directors, agents, employees and volunteers from any all liability, claims and costs arising from or growing out of such treatment. I will not hold the church, or their staff, administration, or workers, liable for any injury to or loss of possessions by the above minor during any activity either on the church property or away, including regular meetings as well as special events. I understand that any off-property AWANA excursions will be communicated with me beforehand and I will be required to sign a separate medical release form in order for my child to participate in these excursions. A 2-4 year old Puggles and Cubbies Parent must remain in the building during AWANA clubs. They must be picked up at the end of the cub by their parent unless we are informed differently by the parent. All Sparks and T&T clubbers must be dropped off and picked up in the AWANA room. If there is someone that is not allowed to pick up your child, please notify our AWANA staff. Once your child leaves the AWANA Club, their actions are your responsibility. I have read and agree to the Terms and Conditions stated above.

Photo Release: Photographs are sometimes taken of Awana ministry activities for publicity and promotional purposes, which include, but are not limited to, in-house presentations, church web site, brochures and newsletters. By signing below, you are granting the church to use photographs of the above mentioned minor(s) as stated.
